Complete this form to request Military OneSource representatives, military and family life counselors (MFLC), child and youth behavioral counselors (CYB-MFLC) and personal financial counselors (PFC) for 1-3 day events. Eligible events include Yellow Ribbon Reintegration Program and other deployment support, drill weekends, family events, annual training, and marriage enrichment retreats. Services Requested

Military Family & Community Policy (MC&FP) and Office of Financial Readiness (FINRED) provides various confidential non-medical counseling and informational resources for Unit support.

A request can only be submitted for either Personal Financial Counselor, or any combination of Military and Family Life Counselor, Child and Youth Behavioral Counselor, and Military OneSource. Please submit an additional request form to request all needed resource types for your event.

* Please select the services for your event, select either in-person or virtual, and the duration of each service to equal the total number of service hours requested. You can reflect actual scheduled briefing or training times within the Additional Information area. If PFC Services are requested, you will also be required to reflect the service type/topic for each category of services requested. For briefings and training, ensure you are requesting an adequate amount of Financial Readiness Training time for the PFC to deliver what you are requesting.

Attendance and Location Information

Please provide event attendance estimates to help us determine the appropriate number of counselors needed for your event.

Attendance Estimates

We expect a minimum of 50 eligible attendees for counseling support. If you have less than 50 attendees, we encourage you to submit your request anyway. We review and staff each request based on its own merit.
